Booking Confirmation
After booking an appointment, here are the next steps:
1. Please login to your Vagaro Account. You will need to confirm your email and contact information.
2. You will receive a Treatment Disclosure Form for the specific treatment you are interested in. This must be completed prior to treatment.
3. When signing the Treatment Form you are agreeing to the following:
Health Screening & Medical Disclosure, Medical Disclaimer, that you do not have any of the existing medical symptoms or chronic conditions listed and have agreed to the Treatment Scope & Informed Consent
4. There are established Results & Expectations for each treatment that will be explained prior to service.
5. After Treatment you will receive a Follow-Up Recommendations Form that shares Client Responsibilities for pre- and post-treatment instructions.
Please understand that Body Contour Lounge cannot control your diet, exercise, hydration, medications, hormone levels, or other lifestyle factors that may affect treatment outcomes.
Personal Property & Safety
Please remove jewelry, watches, body piercings (when applicable), and any metallic objects from treatment areas before treatment begins.
Please follow all safety instructions provided by Body Contour Lounge staff during my appointment.
Cancellation, Refund & Chargeback Policy is listed in the
Treatment Disclosure Form.
